At the end of Guns of Stygia, kill all the enemies on the walkways before the explosions do

Kill a burnout using the hellicopter that arrives in Crash Site
:gold: Heart Attack
Kill a Boss using a Hot Dog cart in the Hotel Elysium map
:bronze: Bell End
Kill 5 enemies at the same time using "Dung" in the Villa map
:silver: Extinguisher
In the Sewer map Kill 5 enemies in 1 wave by setting them on fire then extinguishing them
*got this list after a update (1.01), most likely the DLC will come out today in NA, and EU tomorrow(not 100% sure on release date) *note i got the update and list, on a SCEA(NA) copy of Bulletstorm.
